甲方： 乙方：甲、乙双方在平等、自愿原则下开展英语培训合作，为明确和保障双方在合作期间享有
的权利和义务，经甲、乙双方友好协商，达成如下协议：
1、协议生效期间甲方负责组建英语团队，负责英语团队的管理和运营，乙方有义务提供
支援和协助。
2、甲方业绩提成为当月英语项目实际总收入除去英语项目的专项开支（如英语项目宣传
单、教材费用等）后的50%，甲方需定时、定量发放提成给乙方，当月15日发放上个
成。
3、协议生效后，若甲方从协议生效之日起因业绩问题满三个月不满六个月而提出中止合
作，乙方应按甲方当月应得提成的40%支付给甲方；若甲方从协议生效之日起满六个月不满
一年因业绩问题而提出终止合作，乙方应按甲方当月应得提成的50%支付给甲方。
4、在协议生效期间，若乙方认为甲方的工作状态不能满足乙方的要求，乙方有权中止合
作；但需结清甲方所有应得提成，具体提成方式请按本协议第三条计算。
5、甲、乙双方在提出终止合作前，均需提前半个月通知对方，双方处理好相关事务方可
终止合作。
6、协议生效期间，在未经甲方许可的情况下，乙方不得以“变态外语培训”名称在各分
校开设外语培训班。
7、协议满一年双方均有权根据上一年合作状况决定是否续签协议。协议期间一年最后一
个月所得提成乙方应按甲方当月应得提成的50%支付给甲方。
8、双方在合作期间因培训发生的新问题协议中未规定，双方本着诚信、平等的态度协 商解决，协议自双方负责人签字（盖章）之日起生效，有效期一年。 甲方签字： 乙方签字（盖章）：
